Learn how a new care coordination tool puts patients first

Putting patients first means bringing together healthcare providers across the system to better coordinate care for high-risk patients. Through improved coordination and information sharing, patient goals will guide their care and ensure that they receive the right care in the right place at the right time.

The Care Coordination Tool (CCT) recently deployed in Ontario contains the functionality to support an enhanced approach to patient-centred care planning, by communicating quickly and securely with care providers across different sectors of the health system; thereby allowing clinicians/physicians the ability to manage issues as they arrive. The CCT supports the viewing and sharing of assessment information at the local Health Service Provider (HSP) level. This is especially important when the patient moves from one HSP to another.

This webinar will explore the benefits behind the CCT by offering an overview of the tool, how it functions, and how it will revolutionize healthcare information exchange.
