Communicating Adverse Drug Events: An Imperative for Patient Safety

Emerging evidence indicates there's inadequate information sharing about Adverse Drug Events (ADE) across health care sectors and between care providers which may lead to unintentional re-exposure of patients to medications that previously caused significant harm. The presentation by Dr.Hohl introduces the epidemiology and burden of adverse drug events within our health system, a discussion on why repeat adverse drug events occur and an introduction to the ActionADE pilot project which will help standardize ADE documentation to create ADE reporting and communicate these ADE reports  across health sectors (community pharmacies) to close this information gap.
