The Paper to Electronic MedRec Implementation Toolkit, 2nd Edition highlights emerging evidence for electronic medication reconciliation implementation, and provides resources to support those implementing eMedRec. The toolkit was developed jointly by the Institute for Safe Medication Practices Canada (ISMP Canada), the Canadian Patient Safety Institute (CPSI) and Canada Health Infoway.
